Vancouver Coastal Health (VCH) faces several challenges in delivering effective healthcare services, particularly through its Assertive Community Treatment (ACT) teams:
1. Addressing Complex Mental Health and Substance Use Disorders: ACT teams are designed to support individuals with serious and persistent mental health and/or substance use challenges who have not benefited from traditional outpatient programs. Providing intensive, specific, and continuous services to meet these complex needs is a significant challenge.
2. Impact of Climate Change on Healthcare Delivery: Climate-influenced hazards like extreme heat and flooding are affecting VCH infrastructure, operations, and supply chains. These challenges strain the health system, potentially impacting service delivery.
3. Social Determinants of Health: The Vancouver Coastal Region is home to 14 First Nation communities, each with its own governance structures and capacity to deliver health services. Addressing the unique health challenges and disparities within these communities requires a nation-level approach to creating solutions.

1. Comprehensive, Person-Centered Care: ACT teams provide a holistic approach that combines treatment, rehabilitation, and support services tailored to each individual's needs. This person-centered model ensures that care plans are customized to promote recovery and community integration.
2. Community-Based Service Delivery: Emphasizing care within the community, ACT teams offer an alternative to psychiatric hospitalization. By delivering services in clients' natural environments, they aim to enhance accessibility and effectiveness, fostering stability and independence.
3. Intensive and Continuous Support: Recognizing the challenges faced by individuals with serious and persistent mental health and/or substance use issues, ACT teams provide intensive, specific, and continuous services. This approach is designed for those who have not benefited from traditional outpatient programs, ensuring consistent support to meet complex needs.
